I still remember the first time a dolphin looked me in the eye underwater. I was floating off the coast of Maui, barely twenty feet from shore, and this sleek gray shape just appeared out of nowhere. It rolled onto its side, studied me for a second, then surfaced and exhaled-pfffff-like it was showing off. Then it inhaled in one smooth motion and disappeared.
That exhale stuck with me. Not because it was beautiful (though it was), but because it was effortless. The dolphin didn’t fight for air. It didn’t have a tube in its mouth or a mask strapped to its face. It just breathed, the way nature intended. And I, floating there in my full-face mask, realized something uncomfortable: breathing through gear is not the same as breathing naturally. It’s harder. And sometimes, that extra effort can get you into real trouble.
The Physics You Don’t Think About
When you’re bobbing on the surface, everything seems simple. Put on the mask, stick your face in the water, and breathe. But every time you inhale through a snorkel, your diaphragm has to pull air against the resistance of the tube-and against the pressure of the water pushing on your chest. At just twelve inches below the surface, that pressure adds roughly thirty centimeters of water to every breath you take.
Now add a snorkel that’s narrow, or has a complex valve system, or just wasn’t designed well. Suddenly that easy breath becomes a struggle. Your lungs generate more negative pressure to compensate. And if that negative pressure gets high enough, it can pull fluid from your blood vessels into your air sacs. That’s pulmonary edema. And it can happen fast.
The 2021 Snorkel Safety Study from Hawai‘i tested fifty different snorkel devices and found something alarming: resistance varies wildly, and you can’t tell by looking. Even experienced technicians only guessed correctly about a quarter of the time which snorkels would be high-resistance. So that mask you bought because it looked sleek? You have no idea how hard it is to breathe through until you’re already in the water.
What the Survivors Tell Us
The study also looked at near-drowning survivors. The pattern was striking: they didn’t choke on water. They didn’t panic and swallow a wave. They just got short of breath, felt weak, and faded. That’s Snorkel Induced Rapid Onset Pulmonary Edema-SI-ROPE. And among survivors, 38% were using full-face masks. Of those, a full 90% said the mask was a contributing factor.
This isn’t about blaming a particular design. It’s about understanding that any snorkel adds resistance, and resistance can be dangerous when you’re excited, swimming against current, or trying to keep up with a pod of dolphins. Your heart rate climbs, your breathing gets faster, and suddenly you’re asking your lungs to work way harder than they’re used to. That’s when SI-ROPE can strike.
So What Do We Do Differently?
I’ve been snorkeling for years, and these findings changed how I prepare. Here are the simple, research-backed steps I now follow-and I think every water lover should too.
- Test your gear in shallow water first. Before you head out, spend five minutes floating where you can stand. Breathe normally, then breathe deeper. If it feels restrictive at rest, it will feel dangerous when you’re swimming.
- Stay where you can touch bottom. The data is clear: almost all incidents happened where the person couldn’t stand. If a dolphin leads you into deeper water, stop. Assess. Don’t follow blindly.
- Be cautious with full-face masks if you have any health concerns. The study is sobering. If you have respiratory or cardiovascular conditions, talk to your doctor before using any snorkel gear-especially full-face designs.
- Wait two days after flying. This one surprised me, but the evidence is strong. Long flights expose you to mild hypoxia that can subtly weaken your lung membranes. The Hawai‘i Department of Health recommends waiting at least two to three days after air travel before snorkeling.
- Remove your mask if you feel short of breath. This sounds obvious, but in the moment people keep trying to breathe through the snorkel. Don’t. Take the mask off, roll onto your back, and breathe naturally. Signal for help if needed.
- Watch your buddy-and know what trouble looks like. SI-ROPE doesn’t look like drowning. It looks like someone floating quietly, maybe a little tired. If your buddy says they feel weak or can’t catch their breath, get them out immediately.
- Check your location every thirty seconds. Currents drift you without you noticing. A quick glance at the shore can save you from a long, exhausting swim back.
- Don’t exercise hard while breathing through a snorkel. The study specifically warns against this. If you want to swim fast, take the snorkel out of your mouth. The combination of exertion and resistance is exactly what triggers SI-ROPE.

But we can learn from them. We can choose gear wisely, prepare thoughtfully, and listen to our bodies when they say something’s wrong. That’s the difference between a great day on the water and a tragedy waiting to happen.
