I remember my first real open-water snorkel trip like it was yesterday. I was so focused on spotting sea turtles and navigating the reef that I barely thought about the simple act of breathing through a tube. I figured, hey, I’ve been a scuba diver for years—this is the easy part, right?
Turns out, it’s not always that simple. And the more I’ve learned about what actually happens to your lungs when you’re floating face-down, the more I realize how much we take for granted.
The Physics You Don’t Feel
Here’s something I never considered until I dug into the research: when you’re lying flat on the surface, your chest is under water. That alone adds about 30 centimeters of water pressure right where your lungs sit. Then you add a snorkel. Every time you inhale, your diaphragm has to create a vacuum strong enough to pull air through that tube and overcome that extra pressure.
At rest, it’s no big deal. But the moment you start swimming, or if there’s any chop, your breathing rate goes up. And suddenly, that innocent-looking snorkel can become a real workout for your lungs.
Researchers in Hawai‘i tested 50 different snorkels at a moderate breathing rate—about 3 liters per second. The results were all over the map. Some required very little effort. Others needed more than -5 cm of water pressure to get air in. And here’s the part that blew my mind: when the researchers guessed which snorkels were high-resistance just by looking at them, they were right only about a quarter of the time.
You literally cannot tell by eye. I found that humbling.
The Quiet Mechanism: SI-ROPE
That extra negative pressure, repeated breath after breath, can cause fluid to leak from your blood vessels into your air sacs. It’s called Rapid Onset Pulmonary Edema, or ROPE. When it happens during snorkeling, we call it SI-ROPE. And it’s not drowning in the classic sense—you don’t inhale water. Instead, your own body fills your lungs with fluid, starving you of oxygen.
The sequence is eerily quiet: sudden shortness of breath, fatigue, loss of strength, then a feeling of panic or doom, followed by diminishing consciousness. Often there’s no struggle, no thrashing. Just a slow fade.
The study I mentioned looked at 32 snorkel-related deaths over two years. Nearly half were classified as “very likely” caused by hypoxia from SI-ROPE rather than water inhalation. And here’s the part that really got me: a quarter of those victims were experienced divers or spearfishers. People who knew the water. People like you and me.
What Makes Someone More Vulnerable?
- Recent long-haul air travel. A flight from the mainland to Hawai‘i means hours in a low-oxygen cabin environment. That alone can subtly weaken the delicate barrier between your blood and your lungs. The research couldn’t prove it definitively, but the physiology is compelling. Many experts now recommend waiting 48 hours after flying before snorkeling.
- Underlying heart conditions. In the medical examiner’s review, 44% of victims had cardiac issues—often diastolic dysfunction, a stiffness of the heart muscle that goes unnoticed during daily life but becomes critical under breathing strain.
- High-resistance snorkels. This one you can control. A snorkel with poor airflow design adds resistance. What I Changed in My Own Routine

- Test your gear before you’re in trouble. In shallow water, take slow, deep breaths through your snorkel. If it feels tight at rest, it’ll be worse when you’re swimming.
- Stop at the first sign of breathlessness. Remove the mask immediately. Roll onto your back, breathe slowly, and signal for help if needed.
- Give yourself time after flying. I now wait at least two days before any serious snorkeling after a long flight.
- Don’t assume your experience protects you. Confidence can sometimes make us push harder. Respect the surface as its own environment.
- Know your buddy’s baseline. SI-ROPE is silent. If someone goes quiet, slows down, or seems confused, check on them immediately.
A Final Thought
I still love snorkeling. It’s one of the most direct ways to connect with the ocean, and I’ll never stop doing it. But I approach it differently now—with the same awareness I bring to a dive.
The ocean doesn’t care about your certification card. It cares about physics. And when you combine immersion, negative pressure, and a high-resistance snorkel, the physics can work against you in ways that feel sudden and silent.
