I still remember the first time I felt that tightness in my chest while snorkeling. I was floating over a coral garden off Maui, watching parrotfish nibble at the reef, when suddenly my breathing felt... heavy. Not panicked. Not like I was drowning. Just wrong. I popped my head up, pulled off my mask, and breathed normally. Within seconds, I was fine. I shrugged it off and went back to snorkeling.
Years later, after reading the Hawai‘i Snorkel Safety Study, I realized that moment could have been something far more serious. That subtle shortness of breath might have been the first sign of Snorkel-Induced Rapid Onset Pulmonary Edema-SI-ROPE-a condition that quietly claims more lives than most people realize. And I want to share what I’ve learned so you don’t have to learn it the hard way.
The Hidden Danger in Every Breath
Here’s the thing about SI-ROPE: it doesn’t look like drowning. There’s no thrashing, no coughing up water, no cry for help. What happens is this: when you breathe through a snorkel, especially one with high resistance, your lungs have to pull harder to get air. That extra suction can actually pull fluid from your tiny blood vessels into your lung tissue. Fluid fills your lungs, oxygen drops, and within minutes you can lose consciousness-quietly, without anyone noticing.
The study reviewed 32 snorkel-related deaths in Hawai‘i and found that nearly half were very likely caused by hypoxia from SI-ROPE, not from swallowing water. Another 44% were considered “likely” due to the same mechanism. Only 9% were clearly unrelated. That means the majority of snorkeling fatalities may stem from a cause most people have never even heard of.
What Puts You at Risk?
The research zeroed in on three major risk factors:
- Snorkel resistance. Researchers tested 50 different snorkels and found huge variation in how hard you have to inhale. The scary part? Experienced technicians couldn’t identify high-resistance snorkels just by looking at them-they guessed correctly only about 26% of the time. So you can’t trust your eyes when choosing gear.
- Your health. 44% of drowning victims had heart conditions that increased pressure in their lungs-like undiagnosed hypertension or valve issues. Many were probably asymptomatic on land, but the added load of snorkeling pushed them over the edge.
- Exertion. Nearly all survivors described pushing themselves hard just before things went wrong-swimming against a current, covering long distances, or doing workout-like breathing. The harder you breathe, the more negative pressure builds up.
Rethinking Your Gear Checklist
Most snorkeling safety advice focuses on things like “wear a bright swimsuit” or “don’t touch the coral.” That’s fine, but it misses the real dangers. I’ve rebuilt my personal checklist based on this study, and here’s what I now pay attention to:
Choosing a Snorkel
- Pick a simple design. Avoid snorkels with narrow bores or complicated dry-top valves-they tend to create more resistance.
- Inhale deeply through the snorkel in the store. If it feels restrictive at rest, imagine how it’ll feel when you’re swimming against a current.
- Test your gear in a pool or calm, shallow water before heading out to the reef.
Full-Face Masks: A Word of Caution
The study found that 38% of near-drowning incidents involved full-face masks, and 90% of those users considered the mask a contributing factor. These masks can’t be removed quickly in an emergency. You can’t spit out a mouthpiece to breathe freely. You can’t clear water with a sharp exhale. And valve malfunctions can be serious. Before You Get in the Water
- Assess your health. If you have any history of heart or lung issues, talk to a doctor before snorkeling. What feels fine on land can become dangerous when you’re floating face-down in the ocean.
- Wait after flying. The study recommends waiting several days after long air travel before snorkeling. Prolonged altitude exposure can make your lungs more leaky for a day or two.
- Stay where you can touch bottom. Almost all incidents happened in water over the snorkeler’s head. If you feel short of breath, the single best move is to stand up and breathe normally.
- Swim with a buddy who actually watches you. SI-ROPE victims don’t show obvious signs of distress. If your buddy slows down or seems confused, check on them immediately.
While You’re Snorkeling
- Check your location every 30 seconds. Currents are sneaky and can drift you into deep water without you noticing.
- If you feel unexpectedly short of breath, stop immediately. Remove your snorkel, roll onto your back, signal for help, and get out. Do not try to “catch your breath” by breathing harder through the snorkel-that will only make things worse.
- Avoid heavy exertion while breathing through a snorkel. If you need to swim hard, take the snorkel out of your mouth and breathe normally on the surface.
What This Means for All of Us
The Hawai‘i Department of Health data shows that snorkeling accounts for more visitor drownings than swimming, surfing, or any other water activity. And it’s not because snorkelers are inexperienced. It’s because the real danger is invisible-hidden in the design of gear and the limits of our own bodies.
I still love snorkeling more than almost anything. But I don’t treat it as a low-risk activity anymore. I treat it with the same respect I give any ocean sport: understanding the risks, choosing gear wisely, and listening to my body.
That tightness in my chest years ago? I now know it was a warning. I’m grateful I listened. I hope you will too.
