I’ll never forget the day I ignored my own breathing. I was floating off the coast of Maui, watching a sea turtle glide past, and I felt that familiar tightness in my chest. Not panic-just a slight effort to inhale. But I didn’t want to be the first one back to the boat. So I stayed, breathing harder, until the captain blew the horn. On the ride home, I kept thinking: why did I push through? It wasn’t the current or the cold. It was the unspoken rule that says leaving early means you’re not a strong enough snorkeler.
That moment led me down a rabbit hole of research, and what I found changed how I approach every boat trip. The Hawai‘i Snorkel Safety Study revealed something most of us never hear about: a condition called Snorkel-Induced Rapid Onset Pulmonary Edema, or SI-ROPE. It happens when the negative pressure from inhaling through a snorkel-especially one with high resistance-pulls fluid into the lungs. And here’s the kicker: the traditional boat etiquette we follow doesn’t account for this at all.
Where Boat Etiquette Comes From (And Why It Falls Short)
Most of the rules we follow on snorkel boats were borrowed from scuba diving, fishing, and sailing. “Listen to the captain,” “stay with your buddy,” “don’t go past the reef”-these are all about operational safety, not about the unique physiological demands of snorkeling. But snorkeling is different. You’re face-down, submerged, breathing through a tube that might or might not be working with you. Your body is under physical stress from immersion, and if you’ve flown in recently, your lungs may already be compromised from the mild hypoxia of cabin altitude.
The study tested 50 random snorkel devices and found huge variation in resistance. Some required over -5 cmH₂O of negative pressure to inhale at moderate flow. And get this: when experienced technicians tried to guess which snorkels were high-resistance just by looking, they got it wrong 74% of the time. That means the gear you’re handed on the boat could be making your breathing harder, and nobody would know-not even the crew.
What the Research Actually Tells Us
This isn’t abstract. The study reviewed 32 snorkel-related deaths in Hawai‘i. Of those, 15 were classified as “very likely” caused by hypoxia from SI-ROPE-not from water inhalation. Another 14 were equally likely to be hypoxia or aspiration. In other words, most of these drownings probably didn’t start with water in the lungs. They started with the body’s own fluid leaking into the air sacs because of excessive negative pressure during breathing.
The survivor accounts are chilling. Almost everyone described sudden shortness of breath, fatigue, loss of strength, and a feeling of doom-but no splashing or shouting. That’s the silent nature of SI-ROPE. It doesn’t look like the drowning we see in movies. It looks like someone quietly enjoying the view until they stop moving.
The study also flagged prolonged air travel as a likely risk factor. Hypobaric conditions in planes can subtly weaken the pulmonary blood-gas barrier, making you more vulnerable for days after you land. The researchers recommended waiting several days before snorkeling. Yet how many boat briefings include a question about when you flew in? Almost none.
Rethinking Boat Etiquette for Real Safety
After learning all this, I started changing how I lead groups and join trips. Here are four practical shifts that I believe should become standard on every snorkel boat:
- Test your snorkel resistance before you hit the water. Before anyone jumps in, I have them stand on the swim step, put on their mask and snorkel, and take ten slow, deep breaths. I start every briefing with: “If you feel even a little short of breath, come back to the boat. No questions asked. No judgment. The reef will still be here tomorrow.” I’ve had people thank me later for saying that out loud.
- Ask about travel history. On my trips, I casually ask when people landed. If it’s been less than 48 hours, I gently recommend a shorter session or staying in very shallow water where they can stand up. Most people don’t know that air travel can affect their breathing, and they appreciate the heads-up.
A Story That Stuck With Me
Last year, I led a small group in the Florida Keys. There was a retired physician on board, a guy in his early sixties who’d snorkeled before. During the briefing, I mentioned SI-ROPE and the air travel connection. He looked at me like I’d just solved a puzzle he’d been staring at for years.
He told me he’d seen several older patients come in with unexplained pulmonary edema after snorkeling trips. Their workups showed no heart attack, no aspiration, no obvious cause. “We never connected it to the equipment or the timing of their flights,” he said. “Now it makes perfect sense.” That conversation reinforced something I deeply believe: the safety culture around snorkeling is decades behind the science. A New Etiquette for the Water
I’m not saying we should abandon everything we know. Buddy systems, current awareness, and reef protection are still essential. But we need to add a new layer: respiratory awareness. Here’s a simple checklist I now run through before any boat-based snorkeling session:
- Test your snorkel resistance while on the boat.
- Check your last flight-if less than 3 days, consider a shorter session or no session.
- Establish breathing check-ins with your buddy every 30 seconds.
- Agree that early return is a sign of wisdom, not weakness.
- Know your health-if you have any history of heart or lung issues, get medical advice before snorkeling.
The ocean gives us coral gardens, sea turtles, and moments of pure wonder. But it also demands respect for how our bodies work while we’re in it.
