I learned to snorkel the way most people do: by copying someone who already knew how. Breathe in, breathe out, keep your face in the water, watch the fish. Simple. For years that’s all it was. Then I started reading through old Hawai‘i health reports during a rainy afternoon, and I stumbled onto something I couldn’t unsee.
It turns out that the easy, automatic act of breathing through a snorkel can sometimes work against you in ways that have nothing to do with holding your breath. There’s a condition called snorkel induced rapid onset pulmonary edema-SI-ROPE for short-where the negative pressure created with each inhalation can actually pull fluid into your lungs. Not water from outside. Your own body’s fluid. And by the time you feel something wrong, you may already be in serious trouble.
How I Missed the Signs for Years
Looking back, I can remember moments when I felt suddenly tired while snorkeling-a heaviness in my chest, a need to just stop and float. I always blamed the current or not having eaten enough. Now I wonder if some of those moments were my lungs quietly telling me the resistance was too high. The Hawai‘i Snorkel Safety Study tested fifty different snorkel devices and found that resistance varies wildly, even among similar-looking designs. More unsettling: experienced technicians couldn’t tell which ones were high-resistance just by looking at them. They guessed wrong nearly three out of four times.
That hit close to home. I’d been choosing my gear based on what looked comfortable or what a friend recommended. I never once considered how much force my lungs were generating with every single breath.
What Actually Happens Inside Your Chest
Here’s the simple version: when you’re lying face-down in saltwater, your chest is already under about thirty centimeters of water pressure just from being submerged. Every time you inhale through a snorkel, you add more negative pressure on top of that-like sucking through a straw that’s partly blocked. At ten breaths per minute, the cumulative negative pressure over sixty seconds can exceed 350 centimeters of water. For some people, especially after long flights or with underlying heart conditions, that vacuum effect can cause the delicate membranes in the lungs to leak.
- Shortness of breath that feels different from normal exertion
- Sudden fatigue and loss of strength
- A sense of panic or doom
- Diminishing consciousness without any visible struggle
Those are the warning signs. They don’t look like drowning the way movies show it.
The Air Travel Connection Nobody Talks About
One detail from the study stuck with me more than any other. The data couldn’t prove it statistically, but the physiology strongly suggests that recent prolonged air travel makes the lungs more vulnerable. Commercial cabins are pressurized to about eight thousand feet-mild hypoxia, but for several hours. That subtle stress may weaken the lung membranes just enough that snorkeling a day or two later becomes riskier. Almost all the fatalities in the study were visitors. Most had flown in within the previous three days.
I now wait at least two full days after landing before I put a snorkel in my mouth. It feels like a small thing, but it changes the whole tone of a vacation. I explore the shoreline first. I read. I let my body settle back to sea level before I ask it to breathe against resistance.
Gear That Works With Your Body, Not Against It

- Stay where you can touch bottom. The study found almost all incidents happened in water over the snorkeler’s head. Being able to stand and breathe without gear is a safety net that can’t be replaced.
- Exit at the first whisper of trouble. Shortness of breath, dizziness, or unusual fatigue are not things to tough out. Pull your mask off, roll onto your back, signal for help, and get out. The reef will still be there tomorrow.
A Different Kind of Respect
I still love snorkeling more than almost any other water activity. It’s the closest I can get to flying. But I don’t treat it as a passive thing anymore. Every breath through a snorkel is a physical negotiation between your lungs, your gear, and the pressure of the ocean. Understanding that doesn’t take away the magic-it makes it more real.
The next time you lower your face into the water, take a moment to feel the resistance. Notice how your chest moves. Trust what your body tells you. And if something feels off, don’t hesitate. The water will still be there when you’re ready.
